Teladoc Health Launches ‘Chronic Care Complete’ As Its First Unified Chronic Condition Management Experience

Virtual care provider organization Teladoc Health launched a new service called “Chronic Care Complete” to help consumers manage chronic health conditions. The service provides an array of health monitoring tools, health coaching, and behavior modification resources for consumers diagnosed with pre-diabetes, diabetes, hypertension, overweight or obesity, or mental health disorders.

Starting February 17, 2022, Teladoc is offering the service to health plans and self-insured employers, and will help the plans identify eligible members. Teladoc has not disclosed the names of health plans that have agreed to offer the service.
