Arc Of Yates Adds Health Home Care Management Services

Arc of Yates is expanding its services to provide more support for children and youth in Yates County with the addition of health home care management services. Children’s Health Home Services are Medicaid funded Care Management services that support children/youth under the age of 21 who have chronic health, medical, or mental health conditions and their families.
