Core Solutions Launches AI To Identify Health-Related Social Needs  

Core Solutions launched Core Clinician Assist: SDOH Tracking. The tool uses artificial intelligence (AI) to assess large data sets, identify issues concerning Social Determinants of Health (SDOH) and make actionable information available to care management teams and clinicians at the point of care. The SDOH Tracking tool comes embedded in the Cx360 EHR.  

The company said SDOH Tracking is designed for health care providers focused on value- and measurement-based payment arrangements. Potential users include health plans, population health management organizations, health information exchanges, integrated delivery systems, and primary care providers.
