A Survey of State Medicaid Expansion 1115 Waivers

The Affordable Care Act (ACA) included provisions for the expansion of state Medicaid programs to individuals and families with incomes up to 138 percent of the Federal Poverty Level (FPL). In 2012, the Supreme Court ruled that the expansion must be voluntary—states may choose whether or not to participate. With this ruling, many states have foregone the federal government’s offer to pay for the vast majority of expansion and have not extended the Medicaid program to Americans up to 138 percent FPL.
