In November 2008, Aetna and Partners In Care Corporation (PIC) announced plans to pilot a Patient-Centered Medical Home with New Jersey-based primary care physicians (PCPs). The medical home model emphasizes the role of the PCP in managing and coordinating patient care across a range of care providers, including specialists and hospitals.
