66% of Adults See Physician Within 30 Days of Hospital Discharge
Across all adults discharged from a hospital, about 66% saw a physician for follow-up medical care within 30 days. Of the 34% who failed to see a physician for follow-up care within 30 days, about half had received follow-up care within 90 days and the other half received no community-based follow-up care. Among people who saw a physician within 30 days of discharge, 57.3% had at least one chronic health condition, 13.5% had fair or poor mental health, and 39.0% had fair or poor physical health.
