OPEN MINDS Weekly News Wire Strategic Health Care News As of January 2009, the Centers for Medicare and Medicaid Services (CMS) had approved 787 Special Needs Plans (SNPs), and had released new rules to identify participants and target plan enrollment. The plans provide specialized care to individuals with chronic health conditions or severe disabilities, who are low-income, or who are at-risk of institutionalization. This figure includes 442 dual eligible SNPs, 256 chronic care SNPs, and 89 institutional SNPs.
