March 14, 2005 Arizona State Hospital to Face Federal Funding Sanctions The Arizona State Hospital faces federal funding sanctions for the second time since September 2004, after reviewers found the hospital conducted little or no investigation into the deaths of three patients with serious mental illnesses. The December 2004 review found hospital officials conducted incomplete analyses and did not follow up investigations with preventative policy reforms. Reviewers also reported the following findings, including a lack of documented investigations involving patient escapes; inadequate nutrition and care for some patients; emergency, drug, and other procedures that were not followed by medical staff . . .
