Average Primary Care Conversion to Person-Centered Medical Home Takes 12 to 18 Months
The average length of time needed convert a primary care practice into a person-centered medical home (PCMH) was from 12 to 18 months. In a survey of health care organizations 37% said it took no longer than a year and a half, about 16% said it took a year or less, and 23% said the conversion took up to two years.
