Blue Cross Blue Shield of Michigan Operating Largest Medical Home Pilot in Nation

OPEN MINDS Weekly News Wire Strategic Health Care News Blue Cross Blue Shield of Michigan (BCBSM) announced that it operates the largest patient-centered medical home (PCMH) program in the United States. It includes more than 1,000 physicians working in 300 primary care practices located in 40 Michigan counties. In a PCMH, a patient’s personal physician coordinates care with the patient’s other health care providers to develop comprehensive treatment planning. In addition to operating a PCMH program, BCBSM offers members the “BlueHealthConnection” (BHC) a suite of wellness and care management services, that includes a 24/7 nurse . . .
