Medicare Selects 16 Provider Organizations for Primary Care Housecalls

On April 26, 2012, the federal Centers for Medicare and Medicaid Services (CMS) announced the first 16 provider organizations selected for the Medicare Independence at Home Demonstration that tests the power of primary care housecalls as a way to improve care and cut costs for fee-for-service (FFS) beneficiaries with multiple chronic health conditions, including co-occurring serious mental illness or dementia.
