68 Community Health Centers in Five States to Become Person-Centered Medical Homes
OPEN MINDS Weekly News Wire Strategic Health Care News In May 2009, the 68 health centers in Colorado, Idaho, Massachusetts, Oregon, and Pennsylvania that will become person-centered medical homes were announced. The participant organizations will participate in a four-year, $6 million initiative to help safety net community health centers develop person-centered medical home programs.
