June 24, 2002 Utah Develops Physician Form For Advance Care Planning In April, the Utah Department of Health announced the creation of a new physician's form to translate a patient's living will into a doctor's order. Under Utah's Living Will Act a patient's living will or advance directive is not binding unless it has been transcribed into a doctor's order. The new form would be used when terminally ill patients enter the health care system and would accompany the patient through various medical treatment facilities.
