The 31 states selected to participate in the federal Money Follows the Person (MFP) demonstration in 2007 are not meeting their five-year transition target of transitioning 35,380 Medicaid recipients by 2012 from institutions to community settings supported by home- and community-based long-term care services and supports (HCBS). As of June 2011, 15,818 had transitioned and there were 6,780 current MFP participants living in the community and receiving HCBS.
