Core Performance Measure Set Created For Patient-Centered Medical Homes
The Commonwealth Fund's Patient-Centered Medical Home (PCMH) Evaluators' Collaborative has created and released a core performance measure set to assess the effectiveness of patient-centered medical homes. The measures are combination of two outcome domains—cost/utilization and clinical quality—standardized to enable meaningful comparisons between PCMHs. The core measures for adults address the following: Avoidance of antibiotics for acute bronchitis Adult weight screening and follow-up Medication management with people with asthma, for those taking antidepressants, and annual monitoring for any patients taking long-term maintenance medications Breast cancer, cervical cancer, and Chlamydia screening for women Colorectal . . .
