90% of STAAR Initiative Hospitals Form Review Teams With Community-Based Colleagues To Prevent Avoidable Rehospitalizations
The majority of the 148 hospitals (90%) involved in the State Action on Avoidable Rehospitalizations (STAAR) initiative have formed teams with their community-based partners to review rehospitalizations. Prior to enrolling in STAAR, none of the hospitals had established a mechanism to routinely review rehospitalization events with their community-based colleagues. Participation in the STAAR initiative is entirely voluntary and none of the hospitals or their partner organizations receives funding from the initiative.
