Colorado Multi-Payer Medical Home Pilot Reports Reductions In Emergency Visits & Hospitalizations
A preliminary analysis of the three-year Colorado Multipayer Patient-Centered Medical Home (PCMH) Pilot reported a significant reduction in emergency department visits and hospital admissions among the group of 100,000 patients involved. However, the pilot participants—six health plans, the state’s high-risk pool carrier, and 16 family or internal medicine practices—also reported numerous reimbursement obstacles.
