11 Plans Achieve A 5-Star NCQA Health Plan Ranking In 2025
In 2025, 11 health plans received a 5-star rating from the National Committee for Quality Assurance (NCQA), and another 55 plans received a 4.5-star rating in the annual health plan rating report. The ratings are based on the plan’s delivery of high-quality care, excellent clinical results, strong member satisfaction, and improved health outcomes. In 2024, NCQA awarded five plans the highest 5-star rating.
In total, of the 998 plans NCQA reviewed, 276 were Medicaid plans, 592 were Medicare Advantage plans, 556 were commercial plans, and 352 were plans sold through the Affordable Care Act (ACA) Health Insurance Exchanges. Of the plans that earned 5 stars, three were Medicare Advantage plans and eight were commercial plans. No Medicaid plans or ACA exchange plans earned a 5-star rating.
The Medicare Advantage plans that earned a 5-star rating were:
- Kaiser Foundation Health Plan, Inc. – Northern California: a Medicare health maintenance organization (HMO) in California
- Kaiser Foundation Health Plan Inc. – Southern California: a Medicare HMO in California
- Network Health Insurance Corporation (NHIC): a Medicare preferred provider organization (PPO) in Wisconsin
The eight 5-star commercial plans were as follows:
- Blue Cross and Blue Shield of Massachusetts, Inc.: a commercial PPO in Massachusetts
- Independent Health Association, Inc.: a commercial HMO/point of service combined plan in New York
- Kaiser Foundation Health Plan, Inc. – Northern California: a commercial HMO in California
- Kaiser Foundation Health Plan Inc. – Southern California: a commercial HMO in California
- Kaiser Foundation Health Plan of the Mid-Atlantic States, Inc.: a commercial HMO in the District of Columbia, Maryland, and Virginia
- UPMC Health Coverage, Inc.: a commercial HMO in Pennsylvania
- UPMC Benefit Management Services, Inc.: a commercial HMO in Pennsylvania
- UPMC Health Plan, Inc.: a commercial HMO in Pennsylvania
Of the plans earning a 4.5-star rating, 14 were Medicaid plans, 17 were Medicare Advantage plans, and 24 were commercial plans. For the 2025 rating, NCQA stated that no data was required of the 352 ACA exchange plans, so none were assigned a star rating. Additionally, 254 plans earned a 4-star rating, and 363 earned a 3.5-star rating, according to the reporting website. Just two plans received one star: Blue Cross Blue Shield of North Dakota, a Medicaid HMO in North Dakota, and Local Initiative Health Authority, dba L.A. Care Health Plan, a commercial HMO in California.
These rankings were reported in Health Plan Ratings 2025, by the NCQA. NCQA’s Health Plan Report Cards list commercial, Medicare, Medicaid and Exchange health plans. Commercial, Medicare and Medicaid plans are rated based on their combined Healthcare Effectiveness Data and Information Set (HEDIS) and Consumer Assessment of Healthcare Providers and Systems (CAHPS) scores and NCQA Accreditation status. NCQA evaluates health plans on the quality of care consumers receive, how happy they are with their care, and health plans’ efforts to keep improving, according to the reporting website.
