U.S. Health Care System Improving Performance for Chronic Care
The U.S. health care system has been improving its performance on national benchmarks related to care for people with chronic illnesses. The national health care benchmarks are developed by the Commonwealth Fund, and compare national data to the top performing states, regions, health plans, hospitals, other providers, and other countries. National data met, or exceeded, the Commonwealth Fund’s benchmarks for the following measures: Hospitalized patients received recommended care for heart attack, heart failure, and pneumonia Children with a medical home Care coordination at hospital discharge Hospital standardized mortality ratios, actual to expected deaths Risk adjusted hospital mortality rates . . .
