Medicaid Patient-Centered Medical Homes Operational In 25 States

Since 2006, 25 states have implemented Medicaid patient-centered medical homes (PCMH). Nineteen of the 25 states that have implemented payment changes are paying providers a per member per-month care management fee to perform the functions of a PCMH; 21 of the 25 states have aligned payment with PCMH qualification standards; and 14 have implemented performance payments. Twenty-one states provide transformation support, but only five make payments to support up-front costs. However, many of the states that have adopted the PCMH approach continue to revise payment models to take advantage of new opportunities created by public policy . . .
