37% Of Older Adults Discharged From A SNF Were Not Recovered After 90 Days Of Follow-Up Home Health Care
About 37.5% of Medicare beneficiaries discharged to skilled nursing facilities and then home after hospitalization did not have functional recovery even after 90 days of home health services. Of those who did not recover within 90 days of home health services, 49.7% had mild frailty and 40.1% had moderate to severe frailty. Functional recovery was defined as discharge from home health services with stable or improved ability to perform activities of daily living (ADL).
