49% Of Hospital & Health System SDOH Initiatives Addressed Multiple Domains

According to a recent review of studies, nearly half (49%) of hospital and health system initiatives to collect social determinants of health (SDOH) data from their health care consumers addressed multiple SDOH domains, such as social and community context, economic stability, and neighborhood factors. About 41% of the systems collected SDOH data once at baseline (41%). About 32% collected SDOH data at multiple points in the consumer care journey. Another 27% did not report the frequency of assessment.
