About 62% of physicians contact their Medicare patients with two or more chronic health conditions between office visits, and 38% do not, according to a survey of 500 physicians. Within the survey group, 46% of physicians were not aware that as of January 1, 2015, Medicare reimburses for non-face-to-face care management for Medicare beneficiaries with two or more chronic health conditions. However, among the survey participants, only 20% were providing and billing for Medicare chronic care management services.
