New Jersey Medicaid Money Follows The Person Program Transitioned 1,675 People To Community Services In 8 Years

On March 30, 2016, the New Jersey Department of Human Services (DHS) announced that since launching the federal Money Follows the Person (MFP) program in 2008, a total of 1,675 people have transitioned from institutions to community homes and supportive services. New Jersey’s MFP program is called “I Choose Home N.J.” (ICHNJ). About 42% (710 people) of those who have transitioned via ICHNJ are people with intellectual or developmental disabilities, 32% (542 people) are older adults, and 25% (423 people) have physical disabilities.
