CRANIUM stands for “cardiometabolic risk assessment and treatment through a novel integration model for underserved populations with mental illness.” It is a type of collaborative care model (CCM) for CMHCs. For a pilot CMHC that implemented CRANIUM, the cost was about $74 per person per year.
