A Clinical Pharmacist-Led Transition Of Care Intervention In The Emergency Department Reduced Subsequent Medication-Related Events More Than Usual Care

People who made an emergency department visit due to a medication-related event (MRE) had fewer subsequent visits related to the same MRE after the emergency department implemented a pharmacist-led transition of care program compared to people who received usual care, according to a recent study. MREs included adverse drug events with or without misuse and medication nonadherence that led to clinical exacerbation. The researchers said the intervention reduced subsequent emergency department visits and hospitalizations for the same MRE, and recommended testing it in subsequent trials to confirm its effectiveness.
