On October 1, 2019, the Alabama Medicaid Agency implemented the Alabama Coordinated Health Network (ACHN) to replace its Medicaid health home program and three other care coordination programs. The ACHN is a single care coordination delivery system intended to link consumers, primary care physician (PCP) groups, and community resources in each of seven newly-defined regions. In this new program, the regional networks and PCP groups are incentivized to achieve quality goals in the areas of childhood obesity, infant mortality rates, and addiction disorders.
