Integrated Mobile Health Care Program Produced Better Chronic Disease Medication Adherence For Individuals With Lung Disease & Heart Disease Transitioning From Hospital To Home
An integrated mobile integrated health-community paramedicine (MIH-CP) program focused on transitions from hospital to home was able to improve the likelihood of chronic disease medication adherence for individuals with lung and heart conditions, compared to those not enrolled in the program. A total of 89% of MIH-CP participants picked up their newly prescribed medications within 30 days of discharge compared to the less than 70% pick-up rate by those that did not participate in MIH-CP.
