On April 12, 2021, Blue Cross Blue Shield of Minnesota (BCBSMN) announce it launched a program with VillageHealth, a DaVita integrated kidney care program, to coordinate care for people with chronic kidney disease (CKD) or end-stage kidney disease (ESKD).  The VillageHealth program integrates services provided by nephrologists and by primary care professionals. The program will include an awareness campaign for primary care professionals to help them improve kidney disease screening; proactively manage consumers with early-stage CKD; and facilitate appropriately-timed referrals to nephrologists.
