CMS Announces New Dementia Care Coordination Program With APM Reimbursement; Applications Due September 15, 2023
On July 31, 2023, the Centers for Medicare & Medicaid Services (CMS) announced plans to launch a new care coordination program for Medicare beneficiaries with dementia - the Guiding an Improved Dementia Experience (GUIDE) Model. The program will go live on July 1, 2024; with applications due by November 2023.
The intended beneficiary population is community-dwelling Medicare fee-for-service beneficiaries, including beneficiaries dually eligible for Medicare and Medicaid, living with dementia.
