Connecticut Medicaid Releases RFP For Wave 3 Of The Person-Centered Medical Home Plus Program

On September 6, 2019, the Connecticut Department of Social Services (DSS) issued a request for proposals (RFP PCMH+W3 RFP_09062019) seeking organizations to participate in Wave 3 of the Medicaid Person-Centered Medical Home Plus Program (PCMH+). Proposals are due by October 25, 2019; the performance period will start on January 1, 2020 and run through December 31, 2021.
