Coventry’s Kentucky Medicaid Complex Case Management Program For Diabetes Increased Screening Rates
A complex case management (CCM) program for diabetes implemented by Coventry Healthcare, a Kentucky Medicaid managed care plan increased the percentage of members with diabetes who received blood sugar testing and other diabetes-related screenings over the rate of a comparison group not enrolled in the program. In 2014, about 84% of members enrolled in CCM received A1C screening for long-term blood sugar control, compared to 83% a group not enrolled.
