Early Results From Vermont Medicare Care Coordination Program Point To Reduced Expenditures
Early results from the Medicare Support and Services at Home (SASH) care coordination program, which launched in Vermont in 2011, indicate lower growth in annual total Medicare expenditures for the participants than for a comparison group who were not enrolled in the program. SASH is a model of care coordination centered on affordable housing sites for older adults.
