EHR-Based Interventions Associated With 17% Lower Risk Of 30-Day Hospital Readmission
Electronic health record (EHR)-based interventions—such as telemonitoring, case management, and medication reconciliation—were associated with a 17% lower risk of 30-day hospital readmissions, according to a recent study. The odds of 90-day, all-cause readmissions were 28% lower when an EHR-based intervention was used. The researchers concluded that the global health system challenge of reducing readmission rates can be addressed using the EHR as a tool for interventions.
