Five Health Systems Testing ‘Age Friendly’ Care Model
During 2018, five health systems in the United States are testing a prototype “age-friendly” care model in their hospitals, long-term care, ambulatory, and post-acute settings. The health systems are Anne Arundel Medical Center, Ascension, Kaiser Permanente, Providence St. Joseph Health, and Trinity Health. The goal is to reduce health care-related safety incidents that affect older adults; ensure that older adults are satisfied with care; and optimize value for consumers, families, caregivers, health care professionals, and for health systems.
