For Dual Eligibles With Dementia, D-SNP Enrollment Associated With Fewer Preventable Hospitalizations & Readmissions

Individuals with Alzheimer disease and related dementias (ADRD) who were enrolled in dual-eligible special needs plans (D-SNPs) experienced lower rates of preventable hospitalizations compared to those in other Medicare Advantage (MA) plans. Specifically, D-SNP enrollees with ADRD had a rate of 12.5 preventable hospitalizations per 1,000 beneficiaries annually, while those in non-SNP MA plans saw a higher rate of 15.8 per 1,000.
