Highmark Health & Allegheny Health Network Launch Social Care Network & Value-Based Arrangement

On June 2, 2022, Highmark Health and Allegheny Health Network (AHN) announced a two-year initiative to create the High-Performing Social Care Network (HPSCN) that will compensate non-profit community-based organizations (CBOs) for providing services to address social determinants of health, such as food insecurity, transportation, and housing. The HPSCN participant organizations will close the loop for referrals and inform the prescribing clinical professional or case worker what happens after the person is referred to a social care provider organization. The pilot runs for two years, from July 1, 2022, through June 30, 2024.
