On January 26, 2021, Monogram Health announced that Humana extended its contract for kidney disease care coordination services to include Humana members in Tennessee and Alabama. The services are for eligible Humana Medicare Advantage and commercial health plan members with chronic kidney disease (CKD) or end stage renal disease (ESRD). This is an expansion of an agreement announced in December 2019 for Monogram to provide kidney-disease care coordination services for Humana members in Louisiana and Mississippi.
