Humana Launches Medicare Advantage Focused On Chronic Condition Management

Humana is piloting a new care support model to help its Medicare Advantage members manage chronic conditions. The pilot populations are members with multiple chronic conditions and complex congestive heart failure and diabetes in Kentucky, Pennsylvania, and West Virginia. The pilot launched on February 17, 2021. Humana intends to expand the program to more states during 2021.
