Integrated Care Model For Homelessness Linked To 31% Decline In Total Medicaid Costs
A multi-disciplinary integrated care model for people experiencing homelessness resulted in a 30.7% decline in total Medicaid costs, according to a recent analysis. The model provides clinical medical services, behavioral health and addiction treatment services, case management, housing navigation, supportive housing, transportation, and basic needs assistance.
The model was developed by the Illumination Foundation, and was implemented in Orange County, California. From July 2017 through July 2020, the Illumination Foundation served 1,254 consumers with the model.
