Kaiser’s Advanced Care At Home Pilot Program Resulted In Lower Odds Of Acute Care Visits Compared To Traditional Hospital Care
Participants in Kaiser Permanente Northern California's (KPNC's) Advanced Care At Home (ACAH) pilot program had lower odds of acute care visits than those who received traditional hospital care. ACAH offered high-quality care to adults with acute illness at home in two phases: acute and restorative (intensive post-discharge home health care). The program leveraged telehealth monitoring and in-home intravenous antibiotics, phlebotomy, mobile imaging, and assessment by a health care professional.
