The Minnesota Department of Human Services (DHS) plans to cover transitional services for low-income older adults not financially eligible for Medicaid who are transitioning from institutional to community-based settings. The services will be added to the Minnesota Reform 2020: Pathways to Independence demonstration (Pathways), which DHS seeks to extend through January 31, 2030. On August 2, 2024, DHS submitted an extension request to the Centers for Medicaid & Medicare Services (CMS).
