New York Announces Six Care Coordination Organizations For People With I/DD

On March 5, 2018, the New York Office for People With Developmental Disabilities (OPWDD) announced it had selected six provider organizations to provide a new Medicaid care coordination organization/health home (CCO/HH) care management service to people with intellectual/developmental disabilities (I/DD). The selected CCO/HH organizations are Advance Care Alliance; Care Design NY; LIFEPlan; Person Centered Services; Prime Care Coordination; and Tri-County Care. The selected organizations were formed by existing providers of developmental disability services. The CCO/HH services are slated to go live on July 1, 2018.
