Nine Harvard Pilgrim Network Provider Organizations Integrating Behavioral Health Services Into PCMH Models

On November 10, 2014, Harvard Pilgrim Health Care (HPHC) announced that as a result of its most recent quality grant awards, nine HPHC network provider organizations are integrating behavioral health into patient-centered medical homes (PCMH) for primary care or specialty settings. All nine projects are focused on care coordination. Five are focused on behavioral health integration, one is focused on improving health outcomes for patients diagnosed with behavioral health disorders, and three are focused on complex pediatric patients.
