The North Dakota Department of Human Services (DHS) Developmental Disabilities Division (DDD) is seeking federal approval to add a new community transition service to its Medicaid 1915(c) home- and community-based services (HCBS) waiver for people with intellectual/developmental disabilities (I/DD). The new community transition service would help qualifying individuals move from an institution to community living. The service assists individuals transitioning from an institution or another provider operated living arrangement to a living arrangement in a private residence.
