Among Medicare beneficiaries who used Oak Street Health as their primary care provider organization, the number of hospital admissions and emergency department visits declined by 51%, according to internal data collected by Oak Street Health. The company’s 30-day readmission rates were 42% lower compared to Medicare benchmarks.
