Optum Health Home Care Initiatives Improve Access & Reduce Readmissions

Optum Health reported that home health initiatives supporting consumers returning home after hospitalization improved timely care delivery by more than 20% and reduced readmissions. The initiatives also resulted in lower acute care utilization and shorter skilled nursing facility stays, based on results from initial implementation.
