Optum Health Home Care Pilots Improve Access & Reduce Readmissions
Optum Health reported that pilot home health initiatives supporting consumers returning home after hospitalization improved timely care delivery by more than 20% and reduced readmissions. The pilots also resulted in lower acute care utilization and shorter skilled nursing facility stays.
The home health pilots are part of Optum Health’s broader efforts to improve support during transitions of care.
