Project Transition Launches Mobile Care Platform For TennCare’s I/DD Systems Of Support
Project Transition rolled out a mobile, person-centered care technology platform as part of its TennCare Systems of Support (SOS) model for people with intellectual or developmental disabilities (I/DD) and behavioral health challenges. The state selected Project Transition to provide the SOS services in September 2015. Project Transition entered separate contracts with each of the TennCare Medicaid managed care organizations (MCOs). The services went live on October 1, 2015, with a soft launch before Project Transition deployed the Mozzaz platform in May 2016.
