Santa Clara County, California, Selects Telecare For Mental Health Pay-For-Success Project
On February 8, 2017, Santa Clara County, California announced it had selected Telecare for the county’s upcoming mental health pay-for-success (PFS) project, called Partners in Wellness. The project is focused on providing community-based mental health to 250 individuals with serious mental illness (SMI) and complex needs. The goal is to reduce the participants’ jail days, and use of emergency and inpatient psychiatric services. Partners in Wellness is the county’s second PFS project.
